     i). Recent passport size photograph in .jpeg, .png. jpg format (* maximium upload size will be 1MB)
     ii). All the Certificates (MBBS, DCH, DNB, MD, MCI/State Medical Council /Nursing Council Registration, UG/PG, B.sc/M.sc, ANM/GNM) Should be in PDF Format.(* maximium upload size will be 4MB)
